Appointments
No walk-in appointments, please. All NEW patients must be referred by your Primary Care Physician, Specialist, or other medical providers.
Insurance and Billing
We accept the majority of insurances, including Medicare and Medicaid.
Payment Options
Co-pays, co-insurance, and outstanding balances are due at the time of service.

Parking & Entrance
Entrance to our parking lot is via Classen Drive. Please park on the EAST side of the building.
Please do not park on Shartel Avenue.
